Kidney Car Donation Instructions
1. Thank you for making your car a Kidney Car! The entire donation process hinges on your vehicle’s certificate of ownership (title). This is a different document than your registration. See the TITLE INFORMATION for instructions on signing your title. We must have your title properly signed (and not defaced), in order to proceed.

2. Tax Deduction: NKF-MD cannot guarantee any selling price for any vehicle. It may take up to 30 days or longer to sell your vehicle and for you to receive notification of your tax deduction. We will mail you a receipt within 30 days of the sale of the vehicle.
3. Remove Personal Belongings from your Car: Make sure you remove all personal items from the car. Also, ensure that you remove anything that identifies you (i.e. the vehicle manual with your name on it, old registration cards, etc.)
4. Gather Together:
- Your 3 completed forms signed in the designated places:
- Download: Kidney Car Assessment Form
- Kidney Car Information Sheet
- Odometer Statement
- W-9 Form
- Car Title
- Lien Release, if not stamped by MVA (Notice of Security Interest Filing or Letter of Satisfaction from institution), if applicable.
- Mail them to: National Kidney Foundation of MD, 1107 Kenilworth Drive, Suite 202, Baltimore MD 21204-2186, Att: Kidney Car Program Director.
5. License Plates:
- It is your responsibility to return these to MVA on or before the date you cancel your insurance. Failure to do so will result in an Uninsured Motorist penalty fee of $150.00. If you are transferring your tags to a new vehicle, you must keep the insurance on the donated vehicle until it is picked up. We recommend that you cancel your insurance once your car has been towed.
Do not remove your license plates before pick-up arrangements have been made with the towing company AND do not return the tags to the MVA before the vehicle has been towed. The National Kidney Foundation of Maryland will not accept responsibility for your vehicle until it is towed and in our possession.
6. NKF-MD Logo: Please print and display the Logo on the front windshield when you hear from the towing company, as this will identify your vehicle as a "Kidney Car."

7. If Owner of Vehicle is Deceased: We would need the original title and owner's notarized death certificate. If husband and wife are on title as co-owners, the death certificate alone will suffice for the deceased. However, if only one person is on title, a Letter of Administration is also required and the designated Personal Representative must sign the title in his or her name and receipt will be sent in that name.
